Hard Copy Request Form

Some schools try to cut down on the amount of paper they print and send home.  In an attempt to do this, schools are moving more towards electronic communication, however, not all families have access to the internet.  The school may wish to keep a list of parents who indicate that they do not have internet access so that the school can send hard copies home with those children. This form could be used to collect that data from families, and the school can then maintain a list of students who require hard copies.
