This sample form is form the school can adapt to collect necessary medical an emergency release information for students. Schools should adopt (or use the district’s) form which does receive permission for the school to get emergency medical treatment for students. Schools should also carefully consider a process for ensuring these documents can be easily obtained in the case of an emergency.
